Subjective:
complaints of nausea, vomiting (2-3 episodes) and pain in the right quadrant of abdomen – 5 days, weight loss: 5 kg
Consumed two drinks of vodka before symptoms appeared.
Objective:
tired, Examination done; vital signs: T- 38.2°C, P – 80 bpm & B.P. - 160/90
No abnormality detected
Rx injection pantaprazole and domperidone stat dose given, observation -2 hrs
symptoms resides, Review after 48 hours or earlier depending on the severity of the condition.

Subjective:
Severe vomiting (5 episodes), fatigue, persistent pain in the right quadrant of abdomen, itching on the body, dyspnea.
Objective:
Highly distressed, Head to foot examination done: ascites present, vital signs: T-38.2 °C, P – 88 bpm, R – 28 & B.P. - 150/90, Blood test done: LFT - elevated alkaline phosphatase, LDH, AST & ALT.
Provisional diagnosis:
Liver cirrhosis
