Age 65 years

Home maker and lives with grandchildren
Family History:
Father died of colon cancer (64)
Mother:
Stroke (2017)
Medical History:
Non-smoker, non-drinker
Chron's disease
Moderate Anorexia - intermittent weight loss
No regular medication
Mild Anhedonia (loss of interest in daily activities)
Volvulus:
childhood illness
Treatment
27/02/2020 :
Visited Emergency Department for bilious vomiting and mild abdo pain
Had birthday cake previous day
Objective:
General malaise, listless
Examination:
Soft Abdomen, Positive bowel sound
BP 110/70
PR 80
Discharged - food poisoning (Dx)
Treatment:
IV Fluids, Analgesia, Metoclopramide 10mg, given
28/02/2020 :
Returned to Emergency Department:
↑ abdo pain, vomiting, low mood, ↓ appetite, constipation 4 days.
Objective:
Hiccups, Lethargic, fatigue, general weakness, dizziness
Examination:
Bloated abdomen, tenderness
No bowel sounds
Abdo:
tympanic on percussion
BP 90/60
PR 140
Clammy skin
Blood tests:
low potassium, magnesium; ↑ urea, ↓ creatinine
Abdominal X-ray:
coffee bean shape mass. +/- stricture
Diagnosis:
Large bowel obstruction (sigmoid volvulus)
Treatment:
IV Fluids given
IV analgesia
Nasogastric decompression
Plan:
Urgent refer to surgical team
