Subjective:

15 yesterday sudden onset of L lower abdo pain.
Took Valium -> slept
Today pain still present - sharp & constant, worse sitting up, walking or bending
No urinary symptoms; no vomiting or nausea
No loss of weight, bit constipation recently
Last menstrual period Nov 2004
January 2005:
vaginal bleed & associated crampy abdo pains.
Hospital o/patients:
tentative diagnosis: spontaneous abortion
1 wk before Xmas, episode of lower L abdo pain
Objective:
O/E:
Not distressed
BP 140/80
HR 96, reg
Very tender on light palpation of lower L quadrant of abdo.
Vague mass palpable
Assessment:
? diverticulitis
? pregnancy
Treatment:
Dietary advice:
increase water intake
Avoid strawberries/seeds
B-hCG to exclude pregnancy; FBE; ESR

Subjective:
Pain there but 'easier'
No bowel motion since 28/1/'05:
Passed hard stool with bright blood on outside
Objective:
O/E:
no obvious evidence of anal fissure or haemorrhoids
Assessment:
? haemorrhoids

Treatment:
Refer for flexible sigmoidoscopy

Subjective:
Pain worse after eating last night.
Still no bowel action or flatus.
Objective:
Hb 9.3 g/dl. Mild left shift.
Moderately distressed; abdo (left iliac fossa) tender, rigid with guarding
N bowel sounds
