Works as a librarian
Known asthmatic, uses ventolin reliever inhaler
Appendectomy at 15
Father - dec. 75 years (MI)
Mother - dec 72 years (colon cancer)
Not on medication
No known drug allergies

Subjective:
Returned from overseas trip (India) 07/05/19 with fever, sore throat & general malaise
Objective:
Afebrile, no inflammation observed. Advised rest and fluids
05/07/19
Subjective:
Noticed a neck swelling 2 mths ago, not painful, olive size
Objective:
Generally healthy, BMI 23, pulse 80/min, BP 120/80, temperature 37
Neck examination:
anterior neck swelling 2x2 cm non tender and moves with deglutition, no palpable lymph nodes
CVS, RS, ABD. Examinations are normal
Assessment:
Thyroid nodule
Management:
Order Thyroid function tests (TFT)
U/S neck
Review 2 weeks

Subjective:
Sandra worried about results. No new complaints
Objective:
Normal vital signs, no change in the thyroid exam
TFTs normal
US exam → multiple nodules, calcification on both lobes of thyroid
Assessment and management:
Dx: Multi-nodular goiter. Advised surgical review to rule out malignancy.
Sandra reluctant to surgical intervention. Asked for one week to think it over.
Defaulted her appointment and presented today 6/10/19
Subjective:
Complained swelling ↑ size (1 month)
Noticed diff. swallowing (3 weeks) and hoarse voice (1 week)
Patient accepts surgical intervention
Objective:
Thyroid ↑ size, firm with limited mobility, enlarged lymph nodes on R) Side
Plan:
Refer pt to endocrine surgeon for examination of possible malignancy
WRITING TASK
