Eczema since birth (Elocon, Elidel, Bactroban)
Perforated eardrum (2009)

oranges, strawberries, chocolate; cosmetic products

Siblings: brother - 26, healthy; sister - 15, eczema, asthma
Rheumatoid Arthritis (RA) - father's mother and aunts

Mother concerned re:
a swelling at base of the neck for over a month. Tendency to become constipated (1mth)
On Examination:
Ht - 138cm; Wt - 35kg
Vital signs - normal
A few eczema patches on face and arms
Thyroid gland - upper borderline on palpation
Diagnosis:
Goiter?
Plan:
US Neck. Review with results
Elidel cream twice a day, apply freely on face
↑ water intake and fibre in diet

Mother contacted over the phone:
US - T-gland upper limits, homogenous, hyper vascularity + small cyst
Blood tests arranged. Review with results.
21/12/18
Examination:
- no changes. Eczema - improved.
FBC:
Hb - 134 g/L WCC - 7.9 (x 10 g/L)
TFT:
T4 - 11.50 pmol/L (10.0 - 19.0) TSH - 3.24 mIU/L (0.50 - 4.00)
T3 - 5.7 pmol/L (3.50 - 6.50)
Thyroid AB - +++
25-Hydroxy VitD - 35 mmol/L (>50)
Specialist's opinion over the phone:
Check ups every six months, US + TFT - repeat in a year. If symptomatic refer for consultation
OsteVit-D 0.2 ml daily

Flare-up of eczema after school swimming program: itchy, crusty skin around lips and eyelids
Symptoms of mild hypothyroidism:
↑ tiredness; cold limbs. Constipation improved with diet
On examination:
dry inflamed skin on face, some yellowish discharge from cracks
Neck swelling noticeably ↑
Dx:
Infected eczema. Rx Ceflex 250mg x 4 a day; Elocon ointment - twice daily
US Neck, TFT arranged
Review in a week.
