03/6/19 Hurt back gardening.
Took painkillers, went to work, no rest, pain worsening.
Presented 2 days post initial strain.
X-ray:
No disc problems
Diagnosis:
Lower back strain - severe.
Treatment:
Exercise: gentle walking daily - ↑ time / distance
Referral to physiotherapist
Prescribed:
Diclofenac, carisoprodol
15 days off work - certificate provided for employer
Will review in 2 weeks.
17/06/19 Pt moving stiffly; exercises very painful, but compliant.
Walking 10 mins a day.
Further 15 days rest prescribed.
27/06/19 Progressing slowly - ↑ROM - still in pain.
Pain after 20 mins sitting / lying down.
Very tired after exercise. Pt. worried - not used to being inactive.
Further 15 days rest prescribed.
10/07/19 Walking 20 - 25 mins a day.
2 more physio sessions left.
Pt. still in pain.
Prescribed ↑Diclofenac. Pt reluctant - afraid of addiction.
Pt. bored, anxious about baby's arrival.
Doesn't want to lose job. Keen to return to workplace. Employer supportive.
Fit to work certificate issued:
return to work possible with amended duties - no lifting + regular breaks.
Letter to OT requesting assessment of w/p (advise on duties can perform etc.)
