Widowed for the past 10 years

Patient History:

Caesarean section (1954)

Laparoscopic Cholecystectomy (1967)

Hypothyroidism

Osteoarthritis

Diabetes mellitus type 2

Medications:

Levothyroxine 50 mcg alt days (hypothyroidism)

Panadol osteo 2 t.d.s. (osteoarthritis)

Metformin 500mg b.i.d. (DM type 2)

Fell while descending stairs; arrived by ambulance

Suspected hip fracture

Neighbour heard crying for help, found sitting down on the staircase wearing wet clothes

Neighbour contacted the son but couldn't reach him

Objective:

Pulse 70, BP 110/70

Thyroid examination - nodular goitre

Chest and Abdomen - bruises nil, tenderness nil

Musculoskeletal - full range of movement bilateral hips, no evident injuries except a few bruises on right elbow

Ambulates without pain

MMSE score - 22 (Mild dementia- Age-related)

BSL 6.7

ECG normal

Pelvis and hip x-rays - fractures nil, osteopenia noted

FBC, UEC - normal

TFT, HbA1c, MSU tests*

All results available 10/12/2019

Discharge plan:

Discharge: 9/12/19 with no change to reg meds

Letter to GP to follow up results for:

TFT, HbA1c and MSU tests

Geriatric review

Requires assessment from Aged Care Assessment Team (ACAT) re home assistance (prevent future falls) & personal assistance

Referral to endocrinologist for further assessment of osteopenia recommended.
