You are Dr. Ross McIntyre, Cardiothoracic Surgeon, at the Spirit Hospital, Brisbane. Your patient is Mr. Yanlin Ma who underwent emergency cardio-thoracic surgery on 31/03/19. As an international student, he does not have a current GP so you are referring him the University Medical Centre.

No known allergies
No previous surgery
Reports high blood pressure since late 2016

Smokes 5-6 cigarettes per day

Father died of aortic aneurysm at age 44
Medications:
Panadeine Forte for headaches
Hospital Admission 31/03/19:
Presented to Brisbane Hospital ED with severe chest and back pain
CT scan showed severely dilated ascending aorta and type-A dissection
Transferred to Spirit Hospital. In acute pulmonary oedema on arrival
Echocardiogram performed, showing AI and aortic valve incompetence
Procedure:
Open-chest surgery for repair of dissection and aortic root replacement with mechanical valve
Post-operatively:
Hypertensive initially post-op
Blood pressure stabilised by day 3
Satisfactory post-operative recovery
Reviewed by physiotherapist - patient responsive to exercise program provided
For cardiac outpatients appointment at 3 and 6 months post-op
Discharged 09/04/19
Medications at discharge:
Amoxycillin & Clavulanic Acid 875mg/125mg - 1 BD until course complete
Warfarin 3mg - 1 nocte until next blood test
Amlodipine 10mg - 1 BD
Clonidine 100mg - ½ tab BD
Isosorbide mononitrate 60mg - 1 nocte
Metoprolol 100mg - 1½ tabs BD
Ramipril 10mg - 1 mane
Temazepam 10mg - 1 nocte prn
Oxycodone 5mg - 1 QID prn
Panadol 500mg - 2 tabs QID prn
Plan:
Routine wound care
Referred to Queensland Medical Laboratory (QML) for INR monitoring → maintain at 2.5 - 3.5
Patient education on Warfarin therapy
Monitor BP. To be maintained at 120/80 or below
Wean Isosorbide mononitrate and Clonidine when BP stable
Check Hb
Monitor pain management and cease Endone when possible
Education on weight loss and smoking cessation
Review exercise tolerance and referred to Toowong Cardiac Rehabilitation
WRITING TASK
Write referral letter to Dr. Janine McArdle GP, University Medical Centre, St Lucia, summarising Mr. Ma's condition and the follow-up care he requires.
