You are a registrar on the Rehabilitation Ward where Mr Phillips has stayed for 2 weeks. He is moving to a regional area (Mt Tamborine) to live with his sister while he recovers.
Patient Details:
Name: Kenneth Phillips

05/05/1974
Builder for 23 years, own business
Social History:
Divorced 15 yrs ago. Son in UK
No support from ex-wife/son
Lives alone in own house
Good support from younger sister (Nurse)
Medical History:
Interstitial lung disease since 2012
Patient admitted in Spirit Hospital for bilateral lung transplant on 10/01/2020
Patient stayed ICU post operation, and transferred to Thoracic Ward on 20/01/2020
Patient transferred to Spirit Rehabilitation Ward on 15/02/2020
Post-operative condition:
Patient is using NP 2L. O2 Sats 99%. Patient is very anxious, and prefers to use Oxygen all the time. Aim to stop oxygen if O2 Sats >92% RA (Room Air)
Patient is able to walk 20m with 4 wheel walker. Only requires supervision.
Continent. Bowel open regularly
Low listeria diet and thin fluids. Good appetite
Stage 2 PU (Pressure Ulcer) Coccyx. 2Nd daily dressing
Patient complains pain. Using pressure relief mattress and cushion
Current Medications:
Immunosuppressants: Cyclosporine 500mg BD, Tacrolimus 1mg BD (need to be continued)
Prednisone 15mg Daily
Valganciclovir 450mg Daily
Azithromycin 250mg 3 times per week
Voriconazole 250mg BD
Trimethoprim/Sulfamethoxazole 160mg/800mg 3 times per week
Pantoprazole 40mg Daily
Vitamin D 25mcg Daily
Caltrate 600mg Daily
Magnesium Forte 800mg Daily
Panadol 1g QID
Mirtazapine 30mg Nocte
Recent Medical Notes:
17/02/2020
Patient was quite anxious, and complained SOB during mobility
Continues oxygen therapy NP 2L. O2 Sats 98%. Patient is reluctant to reduce oxygen. Complained coccyx PU pain.
Regular Panadol given. Good effect. Dressing changed. Minimal exudate
22/02/2020
Patient accepted to reduce oxygen to NP 1L this morning. O2 Sat 97%
Patient was able to walk with SPS (Single Point Stick) around 20m W/O SOB
Only required supervision for mobility. Patient was very happy with progress, and keen to be independent
Education about home oxygen provided. Patient reluctant but agreed to go home without oxygen. Patient commenced self-medicating with supervision

Coccyx wound reviewed. Requires dressing every second day after discharge. Aim for discharge 01/03/2020
His sister has offered to visit daily and help with wound dressing change and medication
