10/01/1997 : headache, 30 min - vomiting 4-5 times/day

2-day history of aura 3-5 min preceded by photophobia - phonophobia

01/02/1998 : migraine attack - varieties of symptoms

Ibuprofen - vicodin 50 mg

15/10/2010 :

migraine improved - no more attacks - low mood

unable to move RT eye, painful movement of both eyes more at RT eye

double vision in both eyes

upper respiratory tract infection week ago

No fever, neck stiffness, SOB, nausea, vomiting, urinary, joint pain, ear symptoms

Objective:

BP 127/83, Temp 37.6, RR 16, Pulse 80, BMI 31

RT eye ptosis - no eye protrusion

chest:

normal

heart:

normal

Neurological examination:

fluent, clear speech, sensation, power, reflexes NAD

no signs of eye infection

Plan:

refer to neurologist for urgent neurological assessment
