1998 chicken pox, mumps
2005 RTA
2016 viral fever

Subjective:
complaints of persistent frontal headache, nausea & blurring of vision for 6 hours, no family and history of headache.
Objective:
Physical examination done: No photophobia, neck stiffness.
Visual acuity – normal; no other abnormal changes detected.
Advised to rest & given tab. Paracetamol q4h.

Visited the clinic again with severe left sided throbbing frontal headache along with Blurred vision, 3 episodes of vomiting & right sided parasthesia.
Objective:
Distressed, Vital signs: T-36.7; p-112 bpm & B.P-160/95, Peripheral nervous system Normal.
Rx:
Stat-pethidine 100 mg, intramuscular injection maxolone 10 mg given.
Plan for review after 24 hrs if condition persists.

Home visit
Subjective:
fell down at home after 3 hours of excruciating left sided headache.
Complaining of parathesia in right arm and leg, impaired LOC & Slurring of speech.
Objective:
P-100 bpm, B.P-155/90, Increased knee jerk on right knee, reflexes-normal.
Assessment:
Probable intracranial pathology, space occupying lesions.
Plan:
Needs urgent admission in emergency department.
WRITING TASK
