Father - Died in RTA (40 Years)
Mother - Died in Childbirth
Unknown history for grandparents
Brother – Rheumatoid Arthritis (RA)
PMx:
Low birth weight
Inguinal Hernia (2008)
Annual Influenza Vaccine
Previous diagnosis of Gout - lost to follow up (2009)
Medications:
None
Previously Offered NSAID'S – Stopped by patient
Treatment record:
20.4.19 : 2 weeks history of cough and cold symptoms
Found to be febrile, WCC = 18,000
Started on 7 day course - Amoxyclav PO
Resolved symptoms on follow up
20.5.2019 : Attended for follow up visit

30.6.19 : 2 Weeks H/O Swollen knee joint (Rt)
Joint Aspiration – Suggestive of gouty arthritis
Started on Colchicine 500mg TDS x 2 weeks

Neutrophils 11,000 (78%)
X Ray Left Toe:
- Mouse bitten appearance
Writing Task:
Using the information above, write a letter of referral requesting an urgent appointment to Dr Wysozynski, Consultant Rheumatology, Kentucky Hospital, USA. Make sure you mention secondary prevention in view of recurrent attendances.
