Never smoked
Binge drinking 17 units of ETOH/weekly
Observations:
BP 110/80 mmHg HR 76 bpm
RR 16/MIN
Afebrile to touch
FHx:
Father - Died in RTA (40 Years)
Mother - Died in Childbirth
Unknown history for grandparents
Brother – Rheumatoid Arthritis (RA)
PMx:
Low birth weight
Inguinal Hernia (2008)
Annual Influenza Vaccine
Previous diagnosis of Gout - lost to follow up (2009)
Medications:
None
Previously Offered NSAID'S – Stopped by patient
Treatment record:
20.4.19 : 2 weeks history of cough and cold symptoms
Found to be febrile, WCC = 18,000
Started on 7 day course - Amoxyclav PO
Resolved symptoms on follow up
20.5.2019 : Attended for follow up visit
Chest X Ray NAD
30.6.19 : 2 Weeks H/O Swollen knee joint (Rt)
Joint Aspiration – Suggestive of gouty arthritis
Started on Colchicine 500mg TDS x 2 weeks
31.6.19 : Patient attended with abdominal cramps and watery diarrhea.
? Drug induced : Advised to continue prescribed medication at BD dose + PRN Loperamide
7.9.2019 : Annual Influenza Vaccine
10.10.19 : Re-attended with swollen left toe

Patient would not live to wait for rheumatology appointment
Wants to be treated by private GP
Patient counseled and referred by urgent Rheumatology appointment (1 week wait)

GOUTY Tophi Left ear, vitals NAD
Left knee (Swollen, red, warm to touch), left toe (swollen, red, warm to touch)
Mild Eczema (face) - has tried new facial
Investigations:
Hb 118/dl (115 – 165/dl)
RBC 4.5 (3.80 – 5.50)
PCV 0.40
MCV 79
WCC 14,000
Neutrophils 11,000 (78%)
X Ray Left Toe:
- Mouse bitten appearance
Writing Task:
