Subjective:
fainted at bathroom at home, semi-conscious, trouble talking, hospitalised by her daughter.
Objective:
cold hands, extreme sweating, BSL: 3.5 mmol/L, vital signs: T-38.2 °C, P - 70 bpm, R-22/mt & B.P. -110/70, IV bolus of 50% dextrose given, observation- 2 hrs, patient stable, stopped glipizide for one week, review at clinic after 1 week, pathology requested: FBS, 2 hrs PPBS, FBE, U&Es, Creatinine, LFTs, Full lipid profile, HbA1c.

Pathology reports received:
FBS - 5.6 mmol/L, 2 hrs PPBS – 7 mmol/L; FBE, U&Es, Creatinine & LFTs-Normal range, Full lipid profile: chol - 6.2(high), Trig - 2.4, LDLC - 3.6, HbA1c: 6.7%.
Review pathology reports with patient
Change in medication recommended
Atorvastatin 10mg changed to 20 mg
Glipizide 5mg restarted
Patient Requested for health education on diabetes and diet management
Refer to dietician for dietary management.
