Parkinsonism, Hypertension

Maternal grandmother - died Colorectal Cancer, 76 yrs

Maternal grandfather-died Heart Attack

PMHx:

Childhood chicken pox;

1950 Typhoid fever

1970 Umbilical Hernia-Umbilical Hernia repair done

1998 Back pain-herniated disc, Rx done

2012 Depression-Tab. Amitriptyline 25 mg t.i.d; Hpercholrestrolemia - Tab. Zocor (simvastatin) 10 mg od.

18/03/2018 Myalgia-six weeks; Rx: tab. Ibuprofen t.i.d & Advised to stop simvastatin. Review after one week.

26/03/2018 Persisting pain whole day specially at night, she takes warm bath to resolve the pain.

Weight loss:

6 kg - 2 months

Highly depressed about life.

10/04/2018 Severe pain continues, high level of alcohol consumption to get rid of pain.

Vital signs checked-T-36.8, P-86 bpm & B.P-160/90

Skeletal radiograghy done:

multiple destructive lyctic lesions present.

Urinalysis:

Bence-Jones protein test-Positive.

Provisional Diagnosis:

Multiple Myeloma .

Treatment plan:

Home visit -2 times/week, she needs help with the Diet plan & Medication (Tab. Amitriptyline 25mg t.i.d).
