No known allergies

Family history:

Father - died of Kidney failure

Mother - asthma

Brother - gout

Past medical history:

2000:

gout - colchicine (Lengout), allopurinol, paracetamol.

2002:

laryngitis.

2003:

appendectomy.

2010:

kidney stones.

01/06/2010 : Severe gout attack - steroid injection, colchicine, allopurinol.

01/09/2010 : Gout attack - Advise Pt re lifestyle changes to stop smoking, reduce alcohol, decrease dietary red meat, drink more fluid.

05/12/2010 : Pt appears well. Successful lifestyle adjustment.

2011:

Depression ( ? due to gout ), flouxetine

2011-2020:

No gout attacks.

13/06/2020

Subjective:

Complains of pain in RT big toe.

Swelling in RT foot, RT big toe.

Pain in Rt flank.

Red colour urine - frequency normal.

Objective:

BP: 120/80, HR: 90, RR: 22, T 37.8

Pt anxious, shortness of breath.

Inflamed, red first RT toe.

Tophus - under RT big toe.

Treatment Colchicine (Lengout) 1 mg

NSAIDS

Plan:

Refer to a Rheumatologist for urgent assessment.

Removal of tophus?.

Refer to a Urologist.

WRITING TASK
