Family Background
Parents divorced (2016)
Lives with mother and step-father. 2 step brothers (aged 5, 7)
Mother:
aged care worker. hx of drug use (methamphetamine), binge drinking
Depression
Step father:
landscape gardener-self employed
Social History:
Difficulty making friends and interacting with others. Has difficulty picking up on social cues (body language, humour)
Repetitive behaviour, spinning and twirling
Attends year 3 primary school
One close friend for 5 years
Likes to play alone, has regular arguments with classmates
Psychiatric History:
No previous assessment
Medical and developmental History:
Vaccinations up-to-date
Developmental milestones:
speech and language assessment at age of 3
URTI (2014)
Tonsillectomy (2015)
Emergency department visit for broken index finger (2019)
Not on regular medications
Disruptive during class
20/01/15 :
Parents brought him to clinic concerned about speech
He can say words, short sentences, but not full sentences
Speech and language assessment done (delayed speech)
Plan:
Arranged sessions with speech therapist
Review after 6 months
15/07/15 :
Visited again for review (Minimal improvement)
Parents reported Jackson sometimes upset when they talk to him
Referred him to audiology department for hearing assessment. NAD
Advised to continue speech therapy with follow up
2015-2020:
Family moved to rural area for work. Returned to Brisbane Jan 2020, Jackson started Year 3 at Weller Primary School.
03/07/2020 :
Subjective
Argument and fist fight with three other children
Teachers concerned about Jackson
School progress reports ↓ over past 2 terms
Parents disturbed with Jackson's disruptive behaviour at home. Anger attacks over very minor events i.e not getting what he wants.
Spends prolonged time alone in room
Plays with one toy only
Objective:
Vague facial expression
No eye contact
Minimal response to questions
Preliminary diagnosis:
ASD (autistic spectrum disorder)
Plan:
Refer to child psychiatrist for full psychiatric assessment
WRITING TASK
