Lives in nursing home, because husband died 2 years ago
Has difficulty dressing herself and washing efficiently
Almost total absence of short term memory
Disorientated in regards to time and place
Rapid mood swings –ranges from overactive to drowsy
Accompanied by daughter Mary
Objective:
BP 140/90, P 80
Assessment:
Severe Dementia
Plan:
Mary given diet and fluid plan
Encouraged to be sociable and join in group activites
Constant supervision required
Patient encouraged to dress/wash herself

Dry mouth
Fails to feed herself when drowsy
