You are a locum doctor at Camberwell GP. You are assessing a 22 year old male with worsening asthma.

Married, Wife lives in Surrey.
Medical history:
Asthma since age 5, 2 previous ITU admissions
Eczema
Smoker – 30 pack years history
Allergies:
Aspirin
Hair Spray?
Dog hair
Penicillin
Medication:
Salbutamol 2 puffs PRN
Rescue pack – Prednisolone 30 mg x 5 days
Aqueous cream LA BD
Family history:
Coronary Artery Disease – father died at 89 years
Psoriasis (sister)
Presenting complaints:
Worsening SOB on exertion
Nocturnal as patient debilitating cough (Main concern as patient of insomnia)
Increased use of Ventolin for symptoms
Treatment Record

Subjective:
Not being able to use his blue inhaler – not satisfied with present technique.
Worsening rash related to Eczema
Objective:
Chest Minimal Wheeze
Peak flow 350mls/min (Ideal 400)
Abd SNT BS+
Nil peripheral oedema
Heart Sounds Normal
Tests:
Chest X Ray NAD
Pulmonary function test + reversibility testing = Good response
Full Blood count ↑ WBC
Diagnosis:
Acute infective exacerbation of asthma
?infectious
?? Poor inhaler technique
??? Occupational
Treatment:
Asthma CNS education of use of Salbutamol Inhaler
Addition of spacer attachment to inhaler
Short course of steroids/antibiotics +/- PPI cover
Discussion about smoking cessation including nicotine patches, chewing gums, vaping etc

Review:
DNA previous appointment as unable to get time off work
Better inhaler technique
Ongoing headaches/wheezing at work
Unable to quit smoking
Tests:
Chest X Ray Normal
FBC Normal
Treatment:
Referral to smoking cessation clinic
Referral to occupational health team
Avoid exposure at work
Use facemask whilst at work
Avoid use of hairspray
If above efforts not responding in 3/52 – consider referral to difficult asthma clinic
WRITING TASK
